DOT Physical vs. Regular Physical: Key Differences

A lot of CDL drivers assume their annual physical from their family doctor covers their DOT requirement. It doesn't — and finding that out the day before a job starts is a stressful situation nobody wants to be in. Here's a clear breakdown of how a DOT physical differs from a regular physical, and why those differences matter. ## The Short Answer A **regular physical** (also called a wellness exam or annual checkup) is a general health assessment. Your doctor checks your overall health, reviews your medications, orders routine labs, and screens for common conditions. A **DOT physical** is a federally mandated medical examination with specific pass/fail standards set by the FMCSA. It's designed to determine whether you are physically and mentally fit to safely operate a commercial motor vehicle. The two exams have different purposes, different standards, and different paperwork. ## Who Can Perform Each Exam This is where most drivers get tripped up. **Regular physical:** Any licensed physician, nurse practitioner, or physician assistant can perform your annual wellness exam. **DOT physical:** Must be performed by a medical examiner who is listed on the **FMCSA National Registry of Certified Medical Examiners**. This is a federal database of providers who have completed FMCSA-approved training and passed a certification exam. Your family doctor cannot certify you for a CDL unless they are specifically registered with FMCSA. If you get a DOT physical from a provider who is not on the National Registry, the certificate is invalid — even if the exam itself was thorough. ## What Each Exam Covers ### Regular Physical A standard wellness exam typically includes: - Blood pressure and heart rate - Height, weight, and BMI - Routine blood work (cholesterol, blood sugar, CBC) - Urinalysis - Review of medications and medical history - Cancer screenings (depending on age and risk factors) - Mental health screening - Immunization review The goal is to catch problems early and manage chronic conditions. There are no pass/fail thresholds — your doctor works with you on whatever they find. ### DOT Physical The DOT physical covers many of the same body systems, but with specific federal standards that determine whether you can be certified: - **Vision:** Must meet 20/40 acuity in each eye and 70-degree peripheral field - **Hearing:** Must perceive a whispered voice at 5 feet in the better ear - **Blood pressure:** Specific thresholds that determine certificate length (or disqualification) - **Urinalysis:** Checks for underlying conditions (not a drug test) - **Cardiovascular exam:** Listening for murmurs, checking pulses - **Respiratory exam:** Checking for conditions that could impair alertness - **Neurological exam:** Reflexes, coordination, history of seizures - **Musculoskeletal exam:** Range of motion, strength, limb function - **Medical history review:** Detailed questionnaire covering dozens of conditions The examiner uses all of this to make a binary determination: certified or not certified. ## The Paperwork Is Different After a regular physical, you might get a visit summary or lab results. There's no standardized federal form. After a DOT physical, the examiner completes two specific FMCSA forms: 1. **MCSA-5875** — Medical Examination Report Form (kept by the examiner and submitted to FMCSA) 2. **MCSA-5876** — Medical Examiner's Certificate (the wallet card you carry and provide to your employer) The examiner is required to submit your results to the FMCSA National Registry within 24 hours of the exam. Your state licensing agency uses this data to update your CDL record. ## Drug Testing: A Common Misconception Many drivers think the urine sample collected during a DOT physical is a drug test. It's not. The urinalysis in a DOT physical checks for glucose, protein, and blood — signs of diabetes, kidney disease, or other conditions. It is a health screening, not a substance screening. DOT drug testing is a separate process governed by different regulations (49 CFR Part 40). It uses a different collection process, a certified laboratory, and a Medical Review Officer (MRO). Some employers require both a DOT physical and a DOT drug test, but they are distinct exams. Alternative Chiropractic offers both DOT physicals and DOT drug testing at our Mooresville location. ## Certificate Length **Regular physical:** No certificate. Your doctor may write a letter or note, but there's no standardized validity period. **DOT physical:** Most drivers receive a 2-year certificate. Drivers with certain conditions (controlled high blood pressure, monitored diabetes, etc.) may receive a shorter certificate — 1 year, 3 months, or 90 days — requiring more frequent re-examination. ## Can Your Family Doctor's Notes Help? Yes — even though your family doctor can't perform the DOT physical itself, their records can be very helpful. If you have a chronic condition (diabetes, sleep apnea, heart disease), bringing documentation from your treating physician can speed up the certification process and help the examiner make an informed decision. For example: - A letter from your cardiologist confirming stable heart disease - CPAP compliance data from your sleep apnea treatment - A1C results and medication list for diabetes management Come prepared and the exam goes faster. ## Why It Matters to Use a Certified Examiner Using a provider who isn't on the FMCSA National Registry doesn't just invalidate your certificate — it can create problems with your employer and your state CDL record. Some drivers have had to repeat the entire exam because their first provider wasn't registered. At Alternative Chiropractic, our examiner is listed on the FMCSA National Registry. We perform DOT physicals every week for CDL drivers throughout the Lake Norman area, so we know the process inside and out. ## Schedule Your DOT Physical in Mooresville, NC **Alternative Chiropractic** 484 Williamson Rd Suite D, Mooresville, NC 28117 **(704) 799-0939** Flat fee: **$75**.
